Form Physical Therapy

I take Medicare/ Medicare Secondary, you are only responsible for any deductible or copayment amounts.  

All other insurances are considered out of network.  Your out of network insurance may cover as much at 60-70% of the costs.  Call your insurance company to find out what your out of network reimbursement is for physical therapy.  My out of network rates are $205 for the first visit and $155 for follow-up visits.  I can provide you with a super bill that you may submit to your insurance for out of network reimbursement.

It is the patients responsibility to understand their unique insurance plan. Please verify whether your plan is in network or out of network, if you have visit limits for physical therapy, require preauthorization, deductible/copayments, etc.
