Should you present with symptoms that fall outside the scope of physical therapy, you will be referred to the appropriate provider.

Currently FORM PT is accepting patients with Blue Shield and Medicare insurance, along with patients on a cash/check basis. Average cost for cash pay patients is about $100 per visit. Patients who are paying out of pocket are able to submit receipts for out of network or health spending accounts.

• Spend up to an hour one-on-one with a Doctor of Physical Therapy.
• Does your insurance limit you? FORM PT won’t. Most insurance will not continue to pay for physical therapy after you’ve met your basic range of motion and strength goals. Have you met all of your goals?
• FORM PT charges low and reasonable fees for the services provided.
